Our dedicated team of Dementia Support Workers are here to provide personalised support, practical guidance, and connect you with local services across Buckinghamshire. We understand that every person’s experience with dementia is unique, and we’re committed to helping you live as well as possible with the right support at the right time.
How we support you
Pre-diagnosis:
Are you worried about your memory? If you have concerns but don’t have a diagnosis of dementia, our Dementia Support Workers can still help. Our new Memory Information Service is here for you if you are worried about your memory, already have a diagnosis of Mild Cognitive Impairment, or are waiting for an appointment at the Memory Clinic.
Post-diagnosis:
Following a dementia diagnosis, we provide ongoing support tailored to your needs. You’ll be matched with a local Dementia Support Worker who will get to know your situation and connect you with the right services, activities, and support in your community. Your Dementia Support Worker will stay in regular contact to check how you’re doing and help you access additional support as your needs change over time.
